Get matched to transplant centers that fit you

Eligibility criteria for America's transplant centers, all in one place for the first time

3 simple steps.

Life-changing outcome.

1. Share your medical profile and preferences

Tell us your medical status and preferences, such as travel ability and types of kidneys you’d accept. HIPAA secured.

2. Get matched to the right transplant centers for you

Based on your clinical profile, our intelligent matching identifies hospitals where you may be most likely listed and transplanted* and not simply the nearest one.

3. Prepare your transplant application

Your dashboard shows what's complete and missing, so your application is evaluation-ready at the transplant centers you choose.

  • "So many of us have no idea how to even go about how to apply to other places…For me, my BMI is an issue here, but not in other places, and I don’t know how to go about rectifying that. So I’ve been on dialysis for 10 years and have just given up. This will definitely open up access for people. This will help me get my foot in the door of a place where I don’t even know how to open the door.”

  • “Narrowing down the best transplant hospital for my particular health needs as a living donor would be critical, I have had previous stomach surgeries so only certain hospitals will consider me to donate. To know which ones would help me in my journey to donate a kidney.”

    Mike G.
Would be donor in Ogden, UT

  • "Not only will this clear up discrepancies about our information, but it can help expedite the whole process because a lot of times, we don’t know where we are on the list.”

OUR TEAM

Built by people who understand from every angle

Our founders bring lived patient experience and professional expertise impossible to replicate.

Dr. Michael Kraus, MD, FACP

Chairman
Dr. Kraus is a leading nephrologist with four decades of clinical leadership, spanning the full continuum from bedside care to international recognition for his work in home hemodialysis and organ procurement.

Dr. Kraus is Professor Emeritus at Indiana University, where he served as Clinical Chief of Nephrology. He previously held the roles of Associate Chief Medical Officer at NxStage Medical, Fresenius Medical Care, and Chief Medical Director of the Indiana Donor Network.

At Kidney Connective, he leads clinical infrastructure design that he knows, from experience, is desperately needed in the United States.

Vanessa Evans, MA

Chief Advocacy Officer
A dialysis patient herself and a nationally recognized patient advocate, Vanessa Evans brings lived experience to the heart of Kidney Connective.

As Director of Advocacy & Communities at Fresenius Medical Care, she supports patient communities at scale. She serves on the board of the Kidney Health Initiative, the partnership between the American Society of Nephrology and the FDA.

At Kidney Connective, she ensures the platform is shaped by the people it serves and grounded in authentic patient need.

Grace Lightfoot, ALMc

Chief Executive Officer
Grace builds health research systems that reach patients at scale. Since 2021, she has led the recruitment and enrollment of 100,000 Americans into placebo-controlled clinical trials, with strong representation among women, minority, and rural populations historically underrepresented in research.
These are the same populations that are disproportionately affected by kidney disease.

At Kidney Connective, she leads access and innovation, directing the matching-algorithm design and longitudinal research that underpins the platform's evidence base.

FOR HEALTHCARE PARTNERS

Reduce burden.
Give your team hours back on intake.

Better-matched referrals

Patients are pre-screened against each center's specific eligibility criteria before they ever reach your desk. No more sifting through referrals that clearly don't fit your program.

Pre-assembled packages

Application materials arrive organized and complete medical history, labs, insurance verification ready for transplant coordinators to review, not reconstruct.

Less time on intake

Fewer inappropriate referrals means your team spends less time on ineligible evaluations and more time on patients who are a real fit for your program.
